Fax Orders

The Green Office also accepts fax orders. To submit an order by fax please provide the following information:

• Company Name
• Your Name
• Phone Number
• Email Address
• Billing Address
• Shipping Address (you must provide a physical address)
• Name, Product Code, and quantity of the items you would like to order.
• Credit card number, type (Visa, MasterCard, Discover, American Express), expiration date and the name on the card.
